Distributor Social - SUPPLIER REGISTRATION FORM

This event is set-up to charge per SPONSORSHIP not per ATTENDEE.  A sponsorship includes up to 5 attendees.  Please place a 1 in the "number of attendees" field.  Increasing this number will increase the amount due by $250/members - $300/non-members.  You will be prompted to list only one attendee name because of the unique way this event has to be set-up. Please list remaining attendees in the "additional info." section.
